How to issue power of attorney

Step 1

Fill in the power of attorney, print it out, date and sign it.

Step 2

Make a copy of the authorizing person’s and the authorized representative’s official identification documents.

Step 3

Send the original power of attorney with copies of the identification document to Swisscard.
